Patient Consent and Acknowledgment

By submitting this form or signing below, I confirm that I have read, understand, and agree to each of the following statements. I am at least 18 years of age and am entering into this acknowledgment voluntarily.

I understand that Global Strategic Solutions, LLC, including its DBAs Magnum Performance Labs and Gold Standard Regenerative ("GSS"), is not a medical provider and does not provide medical advice, diagnosis, prescriptions, treatment, surgery, stem-cell therapy, regenerative medicine, laboratory services, or post-treatment medical care.

I understand that any medical procedure, consultation, prescription, treatment, laboratory product, regenerative medicine service, stem-cell-related service, or surgical procedure will be provided exclusively by independent, licensed medical providers outside the United States, and that those providers are not employees or agents of GSS.

I understand that some regenerative, cellular, biologic, stem-cell, exosome, or related therapies available outside the United States may not be approved, cleared, licensed, or authorized by the U.S. Food and Drug Administration, and that statements about such therapies have not been evaluated by the FDA and are not intended to diagnose, treat, cure, or prevent any disease.

I understand that GSS does not guarantee any result, improvement, outcome, size change, aesthetic result, sexual-performance result, medical benefit, regenerative effect, pain reduction, disease improvement, or cure. I understand that individual results vary substantially, that the results described or depicted in any testimonial or before-and-after content are not typical, and that no particular outcome should be expected.

I understand that all medical eligibility decisions, risks, contraindications, treatment recommendations, informed consent, procedure performance, prescriptions, and follow-up care are the sole responsibility of the treating medical provider.

I understand that medical treatment carries inherent risks, and that obtaining treatment outside the United States involves additional risks, including differences in legal systems, regulatory standards, standards of care, available legal recourse, and the availability of follow-up care. I knowingly and voluntarily assume these risks.

I confirm that I have had the opportunity to consult with my own licensed physician and to ask questions, and that I am responsible for obtaining my own independent medical advice before making any treatment or travel decision.

I agree that I will consult directly with the treating medical provider regarding all medical questions, risks, expected results, alternatives, complications, follow-up care, and post-treatment instructions, and that any medical complaint will be directed to that provider.

I understand that GSS may assist with non-medical coordination, scheduling, travel logistics, communication, and concierge support, but that GSS does not control or supervise the medical provider's clinical judgment or treatment decisions.
